Removing Tissue, Not Relaxing It

Upper blepharoplasty removes excess skin, and where appropriate a small amount of muscle or fat, from the upper eyelid. The incision sits in the natural crease, which is why a well done scar becomes very difficult to see once healed.

It is on this menu because it is the honest answer to a specific problem. When upper eyelid skin has genuinely become excess, no neuromodulator, filler, thread or energy device removes it. Treating hooding with a brow lift injection when the issue is redundant skin produces a raised brow and the same hood.

Done properly the eye looks open and rested rather than different. Done too aggressively it looks hollow and surprised, which is far harder to correct than being conservative in the first place. That is a strong argument for taking less rather than more.

Why the Crease Hides the Scar

The upper eyelid crease forms where the levator muscle attaches to the skin. Placing an incision within that natural fold means the resulting scar sits inside a line that already exists and is concealed when the eye is open.

Eyelid skin is the thinnest on the body, which is why it heals with unusually fine scarring and also why it shows swelling so dramatically in the first days.

The decisive judgment is how much to remove. Taking too little leaves the concern unresolved. Taking too much causes difficulty closing the eye fully, dryness and a hollow appearance, and correcting that is considerably harder than the original operation. Risks include dry eye, asymmetry, delayed healing and, uncommonly, changes in lid position. We will go through all of it before you decide.
