Submental fat sits in two layers, above and below the platysma muscle. Liposuction addresses the superficial layer, and knowing how much of the fullness sits deeper matters, because removing the superficial layer alone will not fully correct a neck where the deeper compartment is the problem.
After fat is removed the skin must retract to reveal the new contour. That depends on elasticity, which declines with age and sun exposure. Where retraction is likely to be poor, removing fat alone can leave loose skin that reads worse than the fullness did.
Fat cells removed do not return, though remaining cells can enlarge with weight gain. Risks include contour irregularity, prolonged swelling, temporary numbness and, uncommonly, injury to the nerve supplying the lower lip. We will go through all of it before you decide.
