Radiesse is calcium hydroxylapatite, a mineral your body already recognizes, suspended in a gel carrier. On the day, the gel provides immediate volume and lift. That is the first mechanism and it is the one you see when you leave.
Over the following months the gel is absorbed and the microspheres remaining behind act as a scaffold that prompts your own fibroblasts to lay down new collagen. That is the second mechanism, and it means your result at month four is being held up by something different from what was holding it up on day one.
The important practical difference from hyaluronic acid filler is that this cannot be dissolved. There is no reversal agent. That single fact is why placement, product choice and conservative dosing matter more here, and why this is a physician injection.

What Calcium Hydroxylapatite Is Doing
Calcium hydroxylapatite is a mineral compound found naturally in bone. In this product it exists as microspheres suspended in a carboxymethylcellulose gel. The gel provides the immediate volume and is metabolised over the following months, while the microspheres remain temporarily and act as a scaffold.
Fibroblasts respond to that scaffold by depositing new collagen around it. The microspheres are eventually broken down into calcium and phosphate ions, which the body handles through normal pathways. What persists is the collagen your own tissue made, which is why the result outlives the material.
Because there is no enzymatic reversal, complications are managed rather than undone. Nodules, visibility if placed too superficially, and vascular events are the risks that matter, and all three are placement dependent. It is generally avoided in the lips for exactly this reason, and we will explain where it is and is not appropriate on your face.
